October's prolo pop-up clinic is now confirmed
For MCSF community members, our first Prolo Pop-Up clinic to be held at MCSF is now open for registration.
For general information on Prolo Pop-Ups and associated documents and forms, see this page on our website.
For registration, an Event has been created through the MCSF Facebook page and is now open. Simply select your preferred time (on Weds. Oct. 10th) and fill out the intake forms ahead of time (found on the Prolo Pop-Up page here) or arrive 15 minutes early to fill them out prior to your visit.
More times are soon to be confirmed in November, at which point we will open up registration to the general public.

It is a bit lengthy, but a good read, if I do say so myself. Ok, I'm biased. This is Dr. Angela and I've been fielding so many questions about Santiam Community Health, what it's doing, why, etc., that I created this article on my professional website to answer all those questions, and more.
